The second learning forum, hosted through the Alzheimer Society of B.C.’s Grant to Improve Dementia Care in BC, will be held in Vancouver next spring, to share and celebrate the learning and successes of the seven grant-funded initiatives.

About the Learning Forum
From Knowledge to Practice brought experts and decision makers together to build on the momentum of previous accomplishments.
In 2005, the conference Transforming Dementia Care in BC: Addressing Gaps and Improving Care created the first real opportunity to look at how we can work together to optimize care for people impacted by dementia.
The result was a commitment to the thousands of British Columbians living with the disease and their caregivers to keep quality of life paramount.

Program Outline
Work is being done province-wide as we begin to turn that promise into action:
- The BC Dementia Service Framework (March 2007) provides a solid foundation for action in the care and support of people living with dementia.
- Funded by the $1-million dollar Grant to Improve Dementia Care in BC from the Ministry of Health, seven innovative initiatives are moving forward that each address gaps in the health system.
From Knowledge to Practice leverages the ongoing Grant initiatives to further generate and spread knowledge, and to consider new models for care practices. Join us. Make a contribution to a transformed system and affect the future of dementia care in BC.
